Hopkins, Va. firm plan Md. centers Patient First offices for 'immediate care' will locate in suburbs

Johns Hopkins announced yesterday an agreement with Patient First of Richmond, Va., to develop a dozen "immediate care" medical centers, stretching from Montgomery County to Harford County, over the next three to four years.

Hopkins will become an investor in Patient First, a privately held, for-profit business, said Dr. John D. Stobo, chairman and CEO of Johns Hopkins HealthCare, the Hopkins entity developing health networks. Stobo declined to disclose the size of Hopkins' investment.

The centers, open 14 hours a day, provide no-appointment care to patients who do not have a regular doctor or whose doctor is unavailable. They also offer related services such as labs, X-rays and pharmacies.

The centers in Maryland will carry the Patient First name, Stobo said. He said Hopkins expects that its name will be used in marketing -- any use is subject to Hopkins' approval -- but plans are not complete.

The deal will increase Hopkins' presence in Maryland suburbs. Already, Hopkins has a medical facility at Green Spring Station in Baltimore County, which contains a Patient First center, and a cooperative arrangement with Suburban Hospital in Bethesda. And it has contractual arrangements with hundreds of specialists and nearly 1,000 primary care physicians outside its East Baltimore campus. Other Baltimore hospitals also have suburban outposts; for example, Sinai Hospital recently announced a center in Carroll County.

Such a suburban presence, Stobo said, is important in assuring that Hopkins will "continue to have access to patients," not just to fill hospital beds but for its teaching and research functions.

Hopkins will be a "preferred provider" for referrals from the new Patient First centers, but the arrangement is nonexclusive, said Stephen A. Storey, senior vice president and chief financial officer of Patient First. That means, he explained, that referrals will also take into account insurance coverage and patient preference as well as Hopkins affiliation.

Patient First has six centers in the Richmond area, and four in the Tidewater area of Virginia, according to Storey. Financing is in place to bring the number to nine in each of those markets, he said.

Also, Storey added, "We are currently negotiating with another group, not Hopkins, to do 15 centers in another mid-Atlantic metropolitan market." He declined to provide further details, other than to say the potential partner was not an academic medical center like Hopkins.

"More than anything, we're looking for entry into new markets," he said.

"We're looking to partner with groups that, in the final analysis, are the "A" player in that market. It could be a payer, it could be a hospital system."

The first new Maryland centers should open in nine months to a year. Storey said the plan was to develop centers "from Anne Arundel and Montgomery, through Howard and all the way around to Bel Air."

He said each center costs about $2 million to build, contains 7,000 to 10,000 square feet of space and employs the equivalent of about 35 people (typically, 55 to 60 people, some working part-time).

Neither ambulatory care centers nor a general suburban strategy is a proven winner for hospitals, said Russ Frank, vice president of business development and corporate strategy for Greater Baltimore Medical Center in Towson. His hospital opened a center in Parkville in 1985 and in Towson in 1986, and closed both in 1989.

He said it is not clear that suburban facilities actually increase admissions for urban hospitals, and that the new centers would face competition from existing facilities and new "fast-track" emergency room services planned by his and other hospitals.

But William H. Wenmark, president of the National Association for Ambulatory Care and owner of several centers in the Minneapolis market, said such centers are growing in numbers and in popularity with managed-care insurers, who see them as a way to avoid costly and inappropriate use of hospital emergency rooms.

Nationally, he said, there are about 12,700 such centers, compared with about 8,400 five years ago.

Walk-in patients can even get their prescriptions filled on their way out at every Patient First location and save a trip to the pharmacy.

The accessibility and convenience of walk-in care providers such as Patient First have made them very popular. A quick poll of our medical malpractice lawyers shows that most of us have been to Patient First as a patient or parent. Many people use Patient First as their primary care provider.

Venue for Patient First Lawsuits

Patient First has one or more locations in all of the major counties in central Maryland in including Baltimore, Anne Arundel, Howard, Carroll, Prince George's and Montgomery counties. Patient First also has one location in Baltimore City. This essentially gives plaintiffs the some ability to pick their venue of choice when bringing a malpractice suit against Patient First. In fact, in a malpractice case filed last year (Sasha Hazlett, et al. v. Patient First Maryland Medical Group, P.L.L.C., et al., Cir. Ct. Balt. City - 24C17002851) the Plaintiffs appear to have engaged wise forum selection by filing in Baltimore City. Defense counsel for Patient First, Pessin & Katz, P.A. filed a motion to transfer venue based on forum non conveniens. The motion was denied, presumably because Patient First has a location and engages in "substantial business" within Baltimore City.

Patient First now provides COVID-19 Virus Testing at its Towson, Maryland, location at 950 York Road. Virus Testing is available only at designated Patient First testing centers and is by appointment only. The test that is offered is the RT-PCR (Reverse Transcriptase – Polymerase Chain Reaction) molecular diagnostic test. The test sample is collected at Patient First and is sent to a reference lab for testing.

COVID-19 Virus Testing is also available at the Patient First centers in Aberdeen, Annapolis, Bel Air, Odenton, Owings Mills, Perry Hall and White Marsh.

Testing appointments can be made on-line at https://www.patientfirst.com/covid-19/covid-19-testing. COVID-19 virus testing helps determine if you currently have COVID-19. It is for patients who are at least 5 years old and currently experience symptoms of COVID-19 or meet any of the screening criteria, which is based on guidance from the Centers for Disease Control (CDC). The screening criteria include meeting one or more of the followings conditions:

· You have at least one of the following:

· Coughing

· Shortness of Breath

· Difficulty Breathing

· OR you have at least two of the following:

· Fever

· Chills

· Sore Throat

· Fatigue

· Body Aches or Muscle Pain

· Headache

· Nausea and/or Vomiting

· Diarrhea

· Loss of Appetite

· Loss of Smell and/or Taste

· You are a healthcare worker or first responder.

· You have had prolonged contact with (e.g. live with or work closely with) a COVID-19 positive person or a person waiting for COVID-19 test results.

· You reside in a nursing home or other congregate living facility.

· You are a cancer patient who requires COVID-19 testing before beginning or resuming treatment.

· You are required by your doctor, hospital, or outpatient surgical center to have COVID-19 testing prior to a planned surgery.

· You are required by your employer to have COVID-19 testing prior to beginning or returning to work.

· You are required by another entity to have COVID-19 testing, such as to attend school or camp, or due to travel/transportation requirements.

Results are typically available in less than 3 days. Patients will receive the test result via text message or through a phone call from a nurse. The option for how the patient would prefer to receive test results is determined during registration. The test result will also be available on the Patient First Portal.

For most insured patients, there is no out-of-pocket expense for COVID-19 testing. The visit will be submitted to patients' insurance. Patient First accepts all major insurance plans, including Medicare and Medicaid. For most self-pay patients, there is no out-of-pocket cost for the exam. There may be a separate bill from the outside lab in some cases. The lab determines what, if anything, it bills the patient.

COVID-19 Antibody Testing is also available at all Patient First centers on a walk-in basis.

All Patient First Medical Centers remain open on a walk-in basis from 8 a.m. to 10 p.m., every day, for patients who have an urgent need for medical or injury care. Appointments are not needed for visits unrelated to COVID-19 virus testing. All Patient First centers have taken steps to provide a safe environment for patients and staff, including requiring all patients and staff to wear masks while inside the center, thoroughly disinfecting surfaces throughout the center at regular intervals, requiring frequent hand washing, and limiting visitors to only essential family members.
